The Collaborative Quality Improvement Plan (cQIP) captures the intent of Cambridge North Dumfries OHT (CND OHT) partners as they work together as an integrated team of partners to address common issues. As our OHT evolves, partner organization will have increased capacity to address issues and effect change.

ALC Days

Alternate level of care (ALC) days expressed as a percentage of all inpatient days in the same period. The CND OHT is working together to reduce the number of days those patients spend in the hospital unnecessarily. "ALC" refers to care that would be better provided in a setting such as long-term care, or home, with support. A lower percentage is better, because it means better care for the patients and less costs for the healthcare system.

Mental Health and Addictions-Related Care

Rates of emergency department visits as first point of contact for mental health and addictions-related care. The CND OHT is working to reduce the number of instances where people are accessing initial mental health and addictions support in the emergency department. A lower rate is better, because it means that people have access to support in the community and/or are getting early support, before they need emergency care.

Preventative Care Screening

The CND OHT is working with our primary care partners to increase the percentage of patients screened for cervical, breast, and colorectal cancer. Screening programs can find cancer earlier, increasing the likelihood of treating cancers successfully and less cost to the healthcare system. A higher percentage is better because it means better health outcomes for patients.
